Key Responsibilities

• Manage the active care of patients by collaborating with medical providers and interdisciplinary team members to establish personalized healthcare goals.
• Facilitate and coordinate patient referrals to specialty clinics, review referral records, and monitor the progress and outcomes of care.
• Serve as the primary clinical contact for patient care and care coordination, proactively identifying and addressing delays or obstacles.
• Perform clinical duties including patient assessment, documentation, education, and administering medications or therapies.
• Maintain compliance with safety, infection control, and competency requirements, ensuring a clean and safe working environment.
• Contribute to team efforts by building rapport with colleagues and participating in department improvements and special projects.

Required Qualifications

• Current unrestricted Registered Nurse license in Alaska or another U.S. state (application for Alaska license required if under license transfer).
• Graduate of an accredited nursing school.
• Basic Life Support (BLS) certification through the American Heart Association.
• Successful completion of clinical competency assessment within three months of hire, and every three years thereafter.
• Knowledge of nursing processes, safety protocols, infection control, and good communication skills.

Preferred Experience / Skills

• Bilingual abilities are a plus.
• Previous experience in care coordination or a similar clinical environment.
• Ability to work independently and multi-task effectively in a fast-paced setting.
• Strong critical thinking, problem-solving, and team collaboration skills.
