Key Responsibilities

• Provide comprehensive physical therapy services to patients in skilled nursing and assisted living facilities.
• Develop and implement individualized treatment plans to improve patient mobility and function.
• Monitor patient progress and adjust therapy programs accordingly.
• Collaborate with a multidisciplinary team to ensure optimal patient care.
• Maintain accurate documentation of treatments and patient responses.
• Adhere to all regulatory and facility guidelines.

Required Qualifications

• Valid Illinois Physical Therapist license.
• State-specific licensure or certification as required.
• Proven experience in providing physical therapy services, preferably in skilled nursing or long-term care settings.
• Strong communication and patient-centered skills.
• Ability to work independently and as part of a team.

Preferred Experience / Skills

• Bilingual abilities are a plus.
• Experience with multiple facility environments.
