Job Title LVN / LPN - LTC/SNF

Location Albuquerque, NM

Schedule Full-time, Nights

Key Responsibilities • Provide direct patient care to residents in a long-term care setting, including administering medications, performing vital signs, and documenting patient information. • Develop and implement individualized care plans to meet the unique needs of each patient. • Collaborate with interdisciplinary teams to coordinate patient care and achieve optimal outcomes. • Educate patients and families on proper wound care, medication management, and other health-related topics.

Required Qualifications • Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of New Mexico. • Basic Life Support (BLS) certification. • Ability to lift, transfer, and position patients as needed.

Preferred Experience / Skills • Experience working in long-term care or skilled nursing facilities. • Knowledge of medical terminology and charting systems. • Strong communication and interpersonal skills.
