Key Responsibilities
- Provide physical therapy services to residents in a skilled nursing facility
- Conduct admission evaluations and document assessments and treatment plans
- Administer treatments and therapies per individualized care plans
- Prepare and update progress notes and treatment documentation
- Manage discharges and communicate treatment progress with the care team
Required Qualifications
- Valid state license as a Physical Therapist
- Proven experience in a skilled nursing or long-term care setting
- Knowledge of patient evaluation, treatment planning, and documentation protocols
- Ability to work independently and collaboratively as part of a team
Preferred Experience / Skills
- Prior experience working in a 45-bed skilled nursing facility
- Strong communication and documentation skills
