Key Responsibilities

• Provide skilled physical therapy services to residents in a Skilled Nursing Facility (SNF) and Assisted Living Facility (ALF).
• Develop and implement individualized treatment plans to improve residents' mobility and functionality.
• Document patient progress and coordinate with healthcare teams to ensure optimal care.
• Assist with establishing therapeutic goals and modifying treatment based on patient responses.
• Maintain compliance with all regulatory and safety standards.

Required Qualifications

• Valid Physical Therapist license in the state of Maryland.
• State licensure or certification in good standing.
• Strong knowledge of physical therapy principles and practices.

Preferred Experience / Skills

• Experience working in SNF and ALF environments preferred.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
