Registered Nurse - Home Health
Location: Portland, OR
Schedule / Job Type: Full-time, Days (8-hour shifts)
Key Responsibilities:
- Provide skilled nursing care to home health patients in NW Portland and surrounding areas
- Visit up to six patients per day, adhering to clinical and safety protocols
- Assess patient needs and develop appropriate care plans
- Document care accurately using Epic and Rover systems, ensuring compliance with Oasis documentation standards
- Collaborate with healthcare team members to deliver comprehensive patient care
- Maintain patient confidentiality and follow infection control guidelines
Required Qualifications:
- Valid RN license in Oregon (pending license accepted)
- Minimum of 1 year of experience in skilled home health nursing
- Basic Life Support (BCLS) certification
- Reliable personal transportation to visit multiple home locations within a 60-mile radius
- COVID-19 vaccination series, including one booster
Preferred Experience / Skills:
- Experience with Oasis documentation system
- Familiarity with EPIC and Rover platforms
- Ability to work independently in a home health setting
