Job Title

Schedule / Job Type
• Full-time, travel assignment, days (Wednesday to Saturday, 8:00 AM – 5:30 PM)

Key Responsibilities
• Provide outpatient physical therapy services to patients in a home health setting within McDowell County
• Manage caseload with a minimum of 5.6 visits per day, including admissions, recertifications, functional assessments, and supervision of PT Assistants (PTA)
• Perform therapy procedures such as functional assessments, patient evaluations, and treatment planning
• Communicate effectively with physicians, caregivers, and families to coordinate patient care
• Supervise PTA and ensure adherence to unit policies and protocols
• Document patient progress in EMR system (Cerner Roadnotes) and maintain accurate clinical records

Required Qualifications
• Minimum of 2 years of clinical experience as a licensed Physical Therapist
• Valid BLS certification
• Experience in home health care setting required
• Ability to work autonomously and manage a caseload independently
• Strong organizational and communication skills

Preferred Experience / Skills
• Previous home health experience strongly preferred
• Familiarity with EMR systems, particularly Cerner Roadnotes
• Ability to adapt to diverse patient needs and work independently in a self-directed manner
