Key Responsibilities

• Provide in-home nursing care to patients within a defined geographic area.
• Conduct assessments and develop personalized care plans in collaboration with healthcare teams.
• Perform patient visits, including clinical evaluations and treatments, as scheduled.
• Handle phone triage and provide patient guidance during on-call shifts.
• Participate in weekend rotations approximately once every four weeks, including weekend work and alternate days off.
• Adhere to required credentialing processes and maintain compliance with home health standards.

Required Qualifications

• Home Health nursing experience.
• Current RN license.
• Experience with Oasis documentation for RN.
• Ability to plan for credentialing, including in-person fingerprinting in Delaware.
• Must be able to start ASAP for scheduled orientation.

Preferred Experience / Skills

• Prior experience in on-call nursing and weekend rotations.
• Strong clinical assessment and patient communication skills.
• Ability to work independently in a home health setting.
