Schedule / Job Type

• Full-time, Days shift, 10 hours per shift, 1 shift per week

Key Responsibilities

• Coordinate and monitor patient care in the Emergency Department to optimize patient throughput and ensure appropriate level of care.
• Collaborate with multidisciplinary teams, including physicians, social workers, and community resources, to facilitate safe and efficient patient treatment and discharge plans.
• Assess patient status using clinical criteria to determine inpatient, observation, or outpatient needs.
• Develop and implement care plans, manage resources, and support discharge planning and referral processes.
• Evaluate patients for medical necessity and coordinate care for individuals with complex needs, including mental health and chronic conditions.
• Maintain thorough documentation within Electronic Medical Records and ensure compliance with clinical protocols.

Required Qualifications

• Associates degree in Nursing.
• Valid, unrestricted Nursing license in the United States, Puerto Rico, or US territories.
• Current Basic Life Support (BLS) certification via the American Heart Association.
• Minimum of 2 years of recent care/case management experience in the Emergency Department.
• At least 2 years of patient care experience in hospital, home health, or skilled nursing settings.
• Proficient in Electronic Medical Records, Outlook, Microsoft Word, and Excel.
• Must have 1 year of travel experience.

Preferred Experience / Skills

• Case management certification (CCM or ACM) within 2 years of hire.
• Experience working with diverse populations including Native American communities.
• Knowledge of care management for patients with mental health or chronic pain issues.
