Key Responsibilities
- Assess, develop, implement, and monitor individualized care plans through an interdisciplinary team process, in collaboration with patients and families.
- Conduct comprehensive patient assessments covering biophysical, psychosocial, environmental, functional, and financial aspects.
- Coordinate care transitions between hospital, home, rehab, and long-term care settings, ensuring continuity of care.
- Serve as an advocate for patients and families to access necessary services and resources.
- Communicate regularly with patients, families, healthcare providers, and community resources to facilitate coordinated care.
- Educate patients and families about diagnoses, treatments, and self-care needs before, during, and after hospitalization.
- Collaborate with external providers, insurers, and community agencies to ensure appropriate resource utilization and discharge planning.
- Maintain accurate documentation of assessments, interventions, and outcomes, and track quality metrics.
Required Qualifications
- Graduate of an accredited nursing program (RN).
- Active Oregon RN license.
- Experience in care coordination, post-acute care, or community health (preferred).
- Strong communication, compassion, and teamwork skills.
- Ability to work independently and handle sensitive patient information confidentially.
Preferred Experience / Skills
- Prior experience coordinating community resources for post-acute hospitalization care.
- BSN or MSN degree.
- Case Management certification.
