Key Responsibilities
- Provide physical therapy services to patients in a skilled nursing facility
- Develop and implement individualized treatment plans
- Monitor patient progress and adjust treatments accordingly
- Document patient care activities accurately and timely
- Collaborate with healthcare team to ensure optimal patient outcomes
Required Qualifications
- Minimum of 1 year of professional SNF (Skilled Nursing Facility) experience
- Valid state licensure as a Physical Therapist
Preferred Experience / Skills
- Prior experience working in skilled nursing or long-term care settings
- Strong communication and patient care skills
