Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting, focusing on orthopedics, cardiac, neuro, pulmonary, diabetes, and other conditions.
• Manage a caseload of approximately 5-6 patients per day, ensuring quality care and patient-centered goals.
• Supervise and collaborate with therapy assistants (PTAs) as needed.
• Document patient progress using Epic electronic health records and ensure compliance with BJC policies.
• Collect and maintain necessary certifications such as BLS.
• Effectively communicate and coordinate with the multidisciplinary team to support patient recovery and mobility.

Required Qualifications

• Valid state license as a Physical Therapist in Illinois (or eligible for license transfer/approval).
• BLS certification.
• Minimum 1 year of clinical experience in a home health or related setting.
• Ability to manage a diverse caseload independently.
• Strong organizational and time management skills.
• Proficiency with Epic electronic charting system.

Preferred Experience / Skills

• Experience with home health therapy delivery.
• Knowledge of adult patient populations, including geriatrics.
• Supervision or coordination experience with therapy assistants.
• Flexibility and adaptability to a team environment.
