Schedule / Job Type

• Part-time, Home Health, 8-hour day shifts, Monday through Friday, every 6th weekend

Key Responsibilities

• Provide physical therapy services to home health patients, focusing on mobility, strength, and functional independence.
• Assess patient needs, develop treatment plans, and document progress using Epic electronic charting system.
• Travel within designated territory (40-80 miles) to patient homes, reimbursed at $0.55 per mile.
• Cover assigned shifts while maintaining compliance with scheduling and RTO (no more than 7 days per 13-week contract).
• Communicate effectively with patients, family members, and care team to ensure quality care delivery.
• Maintain BLS certification and adhere to all home health regulations and policies.

Required Qualifications

• Valid state licensure as a Physical Therapist in MO or eligible for licensure, including compact license if applicable.
• Minimum of 1 year of relevant home health experience.
• BLS certification.
• Epic electronic health record system experience.
• Must be able to complete background check and provide references as specified.

Preferred Experience / Skills

• Previous experience working in home health or outpatient settings.
• Ability to float within scope of practice to meet facility needs.
• Prior experience with travel or flexible scheduling is a plus.
