• Therapist - Occupational Therapist

Schedule / Job Type
• Full-time, 5 days per week, Day shifts (8-hour shifts)
Key Responsibilities
• Provide skilled occupational therapy services to home health patients in Medford and surrounding areas
• Conduct patient assessments and develop individualized treatment plans
• Document patient progress using EPIC/Oasis systems accurately and timely
• Collaborate with healthcare team to ensure optimal patient outcomes
• Travel within a 50+ mile radius, maximizing efficiency with minimal drive time
• Adhere to facility guidelines, including COVID vaccination and booster requirements
Required Qualifications
• Valid occupational therapy license for Oregon
• BCLS certification
• Minimum 1 year of recent skilled home health experience
• Own reliable transportation for travel between patient sites
• Experience with Oasis documentation system
Preferred Experience / Skills
• Prior experience working with EPIC/Oasis electronic health records
• Ability to work independently in a home health environment
• Strong organizational and time management skills
