Key Responsibilities
• Provide skilled physical therapy services within a home health setting in East Portland and surrounding areas.
• Manage a patient load with a 1:1 ratio, visiting up to six patients per day.
• Document patient progress using EPIC and Rover platforms, including Oasis assessments.
• Follow facility protocols including COVID vaccination and booster requirements.
• Maintain timely and professional communication with the healthcare team and ensure safe, effective care delivery.
• Travel within a 60-90 minute radius daily, optimizing routes to minimize drive time.

Required Qualifications
• Valid state licensure as a Physical Therapist.
• BCLS certification.
• Minimum of 1 year of skilled home health experience.
• Own transportation to travel between patient locations.
• Ability to work days and adhere to a schedule of 8-hour shifts.

Preferred Experience / Skills
• Previous experience with Oasis documentation.
• Familiarity with home health care protocols and charting systems like EPIC and Rover.
