Key Responsibilities

  • Provide active management of assigned patients within the outpatient clinic setting, collaborating with interdisciplinary teams to establish and achieve individualized healthcare goals.
  • Coordinate care for high-risk or complex patients through multi-disciplinary conferences and outreach, ensuring timely and effective treatment plans.
  • Manage acute and chronic care needs, perform health maintenance activities, review electronic health record reminders, and educate patients accordingly.
  • Facilitate patient referrals within the organization and external facilities, track progress, and report outcomes to healthcare providers for optimal continuity of care.
  • Perform nursing duties including assessment, documentation, medication administration, patient education, and ensuring a safe and clean clinical environment.
  • Maintain compliance with safety, infection control, and annual competency requirements, while promoting teamwork and effective communication within the clinical team.
  • Participate in emergency response, triage, and special projects as assigned, supporting the delivery of comprehensive healthcare services.

Required Qualifications

  • Active, unrestricted Registered Nurse license in Alaska
  • Bachelor’s degree in Nursing (preferred) or graduation from an accredited nursing program
  • At least 1 year of case management experience within the last 2 years
  • Basic Life Support (BLS) certification
  • Proof of current immunizations, including influenza vaccination, and fingerprints for onboarding
  • Ability to work independently in a fast-paced environment, managing multiple priorities effectively

Preferred Experience / Skills

  • Knowledge of tribal health services and system navigation in Alaska
  • National Case Management certification
  • Strong communication, assessment, and conflict resolution skills
