- Full-time, Day shift, 8:00 AM – 4:30 PM, 5 days/week
Key Responsibilities
- Provide outpatient physical therapy services to patients.
- Conduct assessments, take vital signs, and develop treatment plans.
- Implement home exercise programs and assist patients with lifting and transfers.
- Document patient progress and discharge information using Cerner charting system.
- Ensure compliance with safety protocols, including signs and symptoms of abuse.
- Collaborate with the healthcare team to meet patient care goals.
Required Qualifications
- Minimum 1 year of outpatient therapy experience.
- Valid Illinois Physical Therapy Assistant license (active or pending).
- BLS certification.
- Experience taking vital signs and performing assessments.
- Ability to work within scope to meet facility needs.
- Must have an active Illinois license at the time of application.
Preferred Experience / Skills
- Outpatient clinic experience preferred.
- Experience with Short Term Goals and Discharge planning.
- Knowledge of Signs & Symptoms of physical, sexual, or psychological abuse.
- Strong time management skills.

Additional Notes
Candidates must be local or able to self-arrange housing as housing support is not provided. Floating within the hospital or to nearby affiliates is mandatory for all travelers. The role requires compliance with specific licensing, certification, and documentation deadlines. This position involves working on-site at a critical access hospital with 25 beds, utilizing Cerner charting, and following facility protocols.
