Key Responsibilities

  • Assess and coordinate patient care in the Emergency Department to ensure timely and appropriate treatment and disposition.
  • Collaborate with multidisciplinary teams, physicians, and community resources to facilitate patient throughput and discharge planning.
  • Evaluate patient needs for inpatient admission, observation, or outpatient care using clinical guidelines and criteria.
  • Develop and implement care plans, including referrals, discharge instructions, and resource linkage for patients with diverse needs, including mental health issues.
  • Monitor patient progress, assure safe transitions across care settings, and support cost-effective, high-quality care delivery.
  • Communicate effectively with patients, families, and healthcare team members to ensure understanding of care plans and available resources.

Required Qualifications

  • Associates degree in Nursing (ASN or ADN)
  • Valid, unrestricted RN license in the United States or U.S. territories
  • Current Basic Life Support (BLS) certification from the American Heart Association
  • Minimum of two (2) years of care or case management experience in an Emergency Department or hospital setting
  • Proficiency with Electronic Medical Records (EMR), Microsoft Office (Word, Excel), and Outlook
  • At least one (1) year of travel care experience

Preferred Experience / Skills

  • Case management certification (CCM or ACM), to be obtained within 2 years of hire
  • Experience working with diverse populations, including mental health and chronic conditions
  • Knowledge of clinical care guidelines and resource coordination for complex patients
