Job Title
• LVN / LPN - LTC/SNF

Location
• Keene, NH

Schedule / Job Type
• Days, Part-Time (3 shifts per week, 8.5 hours per shift)

Key Responsibilities
• Provide skilled nursing care to residents in a long-term care or skilled nursing facility setting.
• Monitor patient health and document changes accurately.
• Administer medications and treatments as prescribed by medical staff.
• Collaborate with interdisciplinary team to develop and implement care plans.
• Ensure compliance with facility policies and healthcare regulations.

Required Qualifications
• Valid LVN or LPN license/certification in the state of New Hampshire.
• Prior experience in LTC or SNF settings preferred.
• Strong communication and patient care skills.
• Ability to work efficiently and compassionately in a team environment.

Preferred Experience / Skills
• Experience with medication administration and documentation.
• Knowledge of electronic health records (EHR) systems.
