Job Title
• Therapist - Occupational Therapist

• Minot, ND

Schedule / Job Type
• Full-time, Days shift, 40 hours per week (Monday through Friday)

Key Responsibilities
• Provide rehabilitative services to home health and hospice patients according to established standards.
• Develop and implement treatment, rehabilitation, and safety plans for patients and their families.
• Assess individual and family health needs, hazards, and create coordinated therapy plans.
• Document patient care and ensure compliance with regulations, including accuracy of OASIS assessments and medication reviews.
• Contribute to the safety, education, and maintenance of home health and hospice patients, with special focus on geriatric populations.
• Supervise and evaluate patient progress in accordance with departmental policies and procedures.

Required Qualifications
• Licensed Occupational Therapist in North Dakota.
• Minimum of 2 years of experience as an Occupational Therapist (strongly preferred).
• Must hold a current North Dakota occupational therapy license.
• Ability to provide documentation, including proof of licensure and appropriate skills checklist.
• Recent travel experience is required.

Preferred Experience / Skills
• Cerner electronic medical record system experience strongly preferred.
• Experience working in home health or hospice settings, especially with geriatric populations.
• Knowledge of home health regulations, documentation, and OASIS accuracy and competency.
• Ability to perform medication review and safety assessments in a home setting.
