Key Responsibilities
• Provide patient care in long-term care and skilled nursing facility settings
• Administer medications and monitor patient health status
• Assist with daily living activities and document care provided
• Collaborate with healthcare team to ensure quality patient outcomes
• Follow all safety and compliance protocols
Required Qualifications
• Valid LVN/LPN license in the state of New Hampshire
• EOW (End of Week) availability required
• Local candidates only
• Minimum of 1 year of relevant experience preferred
Preferred Experience / Skills
• Experience working in LTC or SNF environments
• Ability to work independently and as part of a team
• Strong communication and organizational skills
