Schedule / Job Type
- Full-time, Mids shifts, 8 hours per shift, 5 shifts per week
Key Responsibilities
- Provide skilled nursing care to residents in a long-term care or skilled nursing facility
- Cover CNA duties as needed, ensuring continuity of patient care
- Administer medications and treatments following facility protocols
- Document patient information accurately and timely in the EMR system
- Collaborate with healthcare team to deliver quality resident care
- Maintain compliance with all facility and state regulations
Required Qualifications
- Active NY State LVN or LPN license
- AHA BLS certification (additional certifications are a plus)
- Minimum of 1 year of LTC/SNF experience
- Ability to perform CNA duties as needed
Preferred Experience / Skills
- Experience with IV therapy is preferred
- Familiarity with electronic medical records (EMR) systems
