Job Title LVN / LPN - LTC/SNF
Location Rutland, VT
Schedule Full-time, Days
Key Responsibilities • Provide high-quality nursing care to residents in a long-term care and skilled nursing facility setting • Develop and implement individualized care plans to meet the unique needs of each resident • Conduct patient assessments, administer medications, and provide wound care as needed • Collaborate with interdisciplinary teams to coordinate resident care and services
Required Qualifications • Current LVN/LPN license in the state of VT • Basic Life Support (BLS) certification • Ability to lift, transfer, and position residents as needed
Preferred Experience / Skills • Experience working in a long-term care or skilled nursing facility setting • Knowledge of elder care and gerontological nursing principles • Bilingual or multilingual skills (not required but a plus)
