Key Responsibilities
• Provide patient care in long-term care or skilled nursing facilities.
• Administer medications and monitor patient health status.
• Assist with daily living activities and documentation.
• Collaborate with healthcare team to ensure quality patient care.
• Follow facility policies and safety protocols.
Required Qualifications
• Valid LVN or LPN license.
• Experience in long-term care or skilled nursing facilities preferred.
• Ability to work 12.5-hour shifts.
• Excellent communication and patient care skills.
• Ability to work rotating weekends and on Mondays and Fridays.
Preferred Experience / Skills
• Previous experience in LTC/SNF settings.
• Knowledge of electronic health records (EHR) systems.
