Schedule / Job Type

• Full-time, day shifts, Monday – Friday (no weekends required, flexible scheduling available)

Key Responsibilities

• Provide outpatient physical therapy services to adult and geriatric patients in a home health setting
• Conduct patient evaluations and develop individualized treatment plans
• Administer therapy techniques and modalities to enhance patient mobility and function
• Document patient progress using the EMR system, with training provided if needed
• Travel to patient locations within designated geographic areas including Xenia, Fairborn, Beavercreek, Springfield, and Dayton
• Maintain compliance with all required certifications and licensing regulations

Required Qualifications

• Valid state licensure as a Physical Therapist
• Minimum of 1 year of physical therapy experience preferred
• CPR certification required
• Ability to operate EMR systems; training provided for those without prior experience

Preferred Experience / Skills

• Home health care experience is a plus
• Comfortable traveling within a 50-mile radius for nursing roles or 30 miles for radiology roles
• Ability to complete required modules during orientation and annually
