Key Responsibilities
• Provide physical therapy services to residents in skilled nursing facilities (SNF) and assisted living / independent living communities (AL/IL).
• Assess residents' physical needs and develop tailored treatment plans.
• Implement therapy interventions to improve mobility, strength, and overall functional ability.
• Collaborate with healthcare teams to ensure coordinated patient care.
• Document patient progress and adjust treatment plans accordingly.

Required Qualifications
• Valid license as a Physical Therapist in the state of California.
• Proven experience working with SNF and AL/IL populations preferred.
• Strong assessment and treatment planning skills.
• Excellent communication and teamwork abilities.
• Ability to work full-time, day shifts.

Preferred Experience / Skills
• Previous experience in skilled nursing or assisted living settings.
• Knowledge of rehabilitation protocols and documentation standards.
