Key Responsibilities

• Provide physical therapy services to patients in skilled nursing facilities and assisted living/independent living communities.
• Assess patients' physical conditions and develop personalized treatment plans.
• Implement therapy interventions to improve mobility, strength, and overall functional ability.
• Document patient progress and maintain accurate clinical records.
• Collaborate with healthcare teams to optimize patient care.

Required Qualifications

• Valid California state license or eligibility to practice as a Physical Therapist.
• Proven experience in outpatient, SNF, or long-term care settings preferred.
• Strong understanding of patient therapy needs and treatment planning.

Preferred Experience / Skills

• Experience working with elderly populations in SNF and AL/IL facilities.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
