Location: San Ramon, CA
Schedule / Job Type: Full-time, on-site, day shifts

Key Responsibilities:

  • Provide occupational therapy services to patients in a home health setting within designated territories.
  • Assess patient needs and develop individualized treatment plans to promote optimal recovery and function.
  • Document patient progress and complete required clinical documentation efficiently.
  • Collaborate with healthcare team members to coordinate patient care and ensure quality outcomes.
  • Follow all state and organizational guidelines, including adherence to vaccination and licensure requirements.
  • Travel to multiple patient locations as required, including outside designated areas if necessary.

Required Qualifications:

  • Current California Occupational Therapy License.
  • BLS certification.
  • Valid CA driver’s license, auto insurance, and registration.
  • Up-to-date immunizations including TB, MMR, Varicella, Hep B, TDaP, and annual Flu shot.
  • Ability to pass drug screening and physical examination.

Preferred Experience / Skills:

  • Previous home health experience is a plus but not required.
  • Ability to work independently in a dynamic environment.
  • Strong documentation and communication skills.
