Schedule / Job Type
• Days, full-time, 12-hour shifts, 3 days per week

Key Responsibilities
• Conduct comprehensive assessments of patients' physical abilities, movement limitations, and pain levels.
• Develop and implement individualized treatment plans based on evaluations, medical history, and patient goals.
• Deliver evidence-based physical therapy interventions to enhance strength, balance, and mobility.
• Collaborate with interdisciplinary team members to coordinate patient care and progress.
• Monitor patient progress, adjust treatment plans as necessary, and document all care in compliance with regulatory guidelines.
• Educate patients and caregivers on mobility techniques, exercise routines, and equipment use.
• Ensure adherence to safety protocols and facility policies, including documentation standards.
• Supervise physical therapist assistants and aides when appropriate.

Required Qualifications
• Graduate of an accredited Physical Therapy program.
• Current state license to practice as a Physical Therapist.
• CPR certification (preferred or required depending on facility).
• Prior experience in a Skilled Nursing Facility or geriatric rehab setting preferred.
• Strong communication, documentation, and interpersonal skills.
• Ability to work independently and collaboratively within a team.

Preferred Experience / Skills
• Experience working with elderly or rehabilitation patients in a long-term care environment.
• Knowledge of Medicare/Medicaid documentation and compliance standards.
