• Therapist - Physical Therapy Assistant

Key Responsibilities
• Provide outpatient physical therapy services to long-term care patients in a skilled nursing facility setting.
• Assist licensed physical therapists in implementing treatment plans and therapeutic interventions.
• Collaborate with healthcare team members to support patient progress and recovery.
• Maintain accurate documentation of patient assessments, treatments, and progress notes.
• Support patient mobility, functional activities, and rehabilitation goals.

Required Qualifications
• Current state-specific Certified Physical Therapy Assistant (CPTA) license or licensure.
• Ability to work full-time hours within the specified schedule.
• Strong communication and teamwork skills.
• Experience working with long-term care or SNF patient populations preferred.

Preferred Experience / Skills
• Prior experience in a skilled nursing facility environment.
• Ability to work flexible hours within the 7 am to 7 pm window as needed.
• Bilingual skills are a plus.
