Licensed Practical Nurse (LPN) – Long-Term Care

Contract | Full-Time | PM Shift (2:00 PM – 10:00 PM) with Rotating Weekends

Key Responsibilities

• Administer medications to residents in accordance with the 5 Rights of medication administration.
• Monitor residents and report any changes in condition to the charge nurse and attending physician.
• Update family members and other relevant parties regarding changes in resident condition as needed.
• Assist CNAs with answering call lights and supporting daily care responsibilities.
• Report any allegations of abuse, neglect, or misappropriation to appropriate facility leadership.
• Perform additional duties as assigned to support resident care and facility operations.

Required Qualifications

• Must hold and maintain a current Wisconsin LPN license.
• Current CPR/BLS certification required.
• Exceptional accuracy and strong attention to detail.

Preferred Experience / Skills

• 1 or more years of related LPN work experience, preferably in a long-term care or skilled nursing setting.
