Hospice Case Manager – Registered Nurse

Contract | Full-Time | Days | Monday – Friday | Onsite / Field-Based
Key Responsibilities
- Manage a caseload of hospice patients receiving care in a home setting
- Conduct patient assessments and develop individualized plans of care
- Coordinate and communicate with interdisciplinary care teams to ensure continuity of hospice services
- Travel to patient homes throughout a rural service area to deliver direct patient care
- Document patient care activities accurately and in a timely manner using electronic medical records
Required Qualifications
- Active Registered Nurse (RN) license in good standing in the state of Vermont
- Prior experience in hospice case management
- Experience providing hospice care in a home-based setting
Preferred Experience / Skills
- Prior experience traveling and providing care in rural environments
- Familiarity with McKesson or Netsmart EMR platforms
