Key Responsibilities

• Provide nursing services to individuals and families in their residence under the supervision of a registered nurse.
• Perform prescribed individualized patient care as delegated.
• Assist the RN in the preparation, evaluation, and modification of the patient's plan of care as appropriate.
• Observe, document, and report the patient's physical and emotional needs and reactions to care to the supervising RN.

Required Qualifications

• Must hold an active Licensed Practical Nurse (LPN) license in Vermont and New Hampshire, or an unencumbered compact license.
• Graduate from an approved school of practical nursing.
• Minimum of 2 years of experience in a home health setting.

Preferred Experience / Skills

• Experience working within an interdisciplinary care team in a home health or hospice environment.
• Familiarity with patient care documentation and care plan modification processes.
