Contract | Days | Primarily 12-hour shifts with some 8-hour shifts | Every other weekend and every other holiday required

Key Responsibilities

• Provide direct nursing care to residents in a long-term care and skilled nursing setting, with a focus on geriatric and dementia populations
• Manage a patient assignment within established nurse-to-resident ratios on a 51-bed unit
• Utilize electronic medical records (PCC) to document assessments, care plans, and clinical notes accurately and in a timely manner
• Collaborate with interdisciplinary care teams to ensure continuity and quality of resident care
• Complete facility-specific orientation including time with the LEAD team and supervised floor shifts

Required Qualifications

• Active and unrestricted Registered Nurse (RN) license valid in the state of Minnesota
• Experience working in a long-term care or skilled nursing facility environment
• Ability to work day shift, including every other weekend and every other holiday
• Proficiency with or ability to quickly learn electronic medical record systems

Preferred Experience / Skills

• Prior experience working with geriatric and dementia patient populations
• Familiarity with PCC (PointClickCare) EMR platform
• Experience in a SNF setting with similar nurse-to-resident ratios
