Contract | Full-Time | Onsite | Monday–Friday Day Shift with On-Call Rotation
Key Responsibilities
- Perform comprehensive patient assessments and develop individualized plans of care
- Provide skilled nursing care and appropriate clinical interventions in patient homes
- Monitor patient progress and communicate changes to the care team
- Educate patients and families on medications, treatments, and care plans
- Coordinate care with LPNs, therapists, caregivers, and providers
- Complete timely and accurate documentation in EMR and participate in on-call rotation
Required Qualifications
- Active Registered Nurse (RN) license in the state of Colorado or active NLC compact license
- Valid driver's license, reliable transportation, and current auto insurance
- Strong assessment and critical thinking skills
- Ability to work independently and manage a structured caseload
- Clear, professional communication with patients and interdisciplinary team members
- Comfort with documentation and EMR systems
Preferred Experience / Skills
- Prior home health nursing experience
- Experience with HCHB (HomeCare HomeBase) EMR system
