Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient setting
• Complete required clinical documentation accurately and in a timely manner
• Develop and implement individualized treatment plans based on patient evaluations
• Collaborate with the clinical team to ensure continuity and quality of patient care
• Maintain an active Physical Therapist license as required for the assignment

Required Qualifications

• Active Physical Therapist license in the state of Texas (permanent license required)
• Experience in an outpatient physical therapy setting
• Ability to fulfill contract credentialing and quality assurance requirements prior to start
• Candidates who have worked in a permanent or PRN capacity at this facility within the last year are not eligible for this assignment

Preferred Experience / Skills

• Prior experience working in a high-volume outpatient therapy environment
• Strong documentation skills and familiarity with electronic medical records
• Ability to adapt quickly to facility-specific workflows and protocols
