Contract | Monday–Friday, 8:00 AM–5:00 PM with on-call and weekend rotation | Onsite / Field-Based
Key Responsibilities
- Provide skilled nursing care and case management to assigned patients in their place of residence, including admission onto services and ongoing coordination of care through an interdisciplinary team approach
- Observe, assess, evaluate, and plan interventions related to patients' physical and psychosocial statuses, including changes in condition and reactions to treatments or medications
- Oversee care provided by Home Health Aides and CNAs, ensuring compliance with the established plan of care
- Ensure continuity of care through effective communication, information exchange, and case conferences with other disciplines and community agencies
- Maintain accurate, current, and complete patient records in accordance with agency standards and OASIS requirements, with knowledge of current reimbursement rules and regulations
- Participate in on-call rotation, assessing and evaluating patient needs via telephone and making home visits as required
Required Qualifications
- Current RN license in the State of Montana required
- Valid driver's license, appropriate auto insurance, and reliable transportation required
- Ability to participate in on-call and weekend rotation as part of the schedule
Preferred Experience / Skills
- BSN degree preferred
- 2 years of recent acute care, home care, or hospice experience, including clinical skills such as venipuncture, catheterization, wound care, and/or parenteral therapy
- Experience with HCHB (Homecare Homebase) electronic medical records strongly preferred
