Registered Nurse – Hospice (Home Hospice Case Manager)

Contract | Days | 5x8 Hours | 8:00 AM – 4:30 PM

Key Responsibilities

• Serve as a home hospice case manager overseeing a patient census of approximately 35–45 patients
• Coordinate and manage individualized care plans for hospice patients in a home setting
• Conduct patient assessments and collaborate with interdisciplinary teams to ensure quality end-of-life care
• Document all patient care activities accurately using EPIC electronic health records
• Float to other units or locations as needed based on facility needs

Required Qualifications

• Active RN license; Virginia license required to be in hand at time of submittal or valid compact license with proof provided
• Home hospice case management experience is required — no exceptions
• Specialty experience within the last year is required
• BLS certification through the American Heart Association (AHA) required
• EPIC charting experience required
• One supervisory reference required at time of submittal
• Locals are welcome and will be considered at the same base rate

Preferred Experience / Skills

• Prior experience managing a high-volume home hospice patient caseload
• Strong interdisciplinary communication and care coordination skills
• Familiarity with hospice regulatory compliance and documentation standards
