Key Responsibilities:
• Provide high-quality nursing care to residents in a long-term care or skilled nursing facility setting • Conduct thorough assessments of patients' medical conditions, develop and implement individualized care plans, and monitor progress • Administer medications, treatments, and therapies as prescribed by physicians and in accordance with facility policies • Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions
Required Qualifications:
• Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) • Valid certification in basic life support (BLS) and/or advanced cardiovascular life support (ACLS) • Ability to pass a state-approved background check
Preferred Experience / Skills:
• Experience working in long-term care or skilled nursing facilities • Knowledge of medical terminology and ability to document patient information accurately • Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines
