Key Responsibilities
• Provide hospice nursing care to patients in a home setting within Buncombe County.
• Assess patient needs, manage pain, and educate families about care plans.
• Document patient information accurately in electronic medical records.
• Travel to patient homes using personal vehicle, with mileage reimbursement at the Federal rate.
• Ensure compliance with hospice certification and documentation requirements.
• Collaborate with the interdisciplinary team to deliver compassionate, patient-centered care.
Required Qualifications
• Active RN license.
• CPR certification.
• Valid driver’s license and reliable transportation.
• Experience in hospice care or home health preferred.
• Familiarity with Electronic Medical Records (EMR).
• Understanding of documentation necessary for hospice certifications.
Preferred Experience / Skills
• Previous hospice case management experience.
• Home health care background.
