Key Responsibilities

• Perform thorough evaluations to assess patients’ physical abilities and develop personalized treatment plans
• Provide physical therapy interventions to improve strength, mobility, and functional abilities
• Educate patients and their families on therapeutic exercises, safe movement techniques, and home modifications
• Collaborate with the interdisciplinary team to coordinate care and optimize patient outcomes
• Monitor patient progress and adjust treatment plans as necessary to ensure the best possible outcomes
• Maintain accurate and timely documentation of patient assessments, progress notes, and treatment plans in accordance with facility policies

Required Qualifications

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program
• Valid Physical Therapy license in the state of California
• Knowledge of physical therapy techniques, therapeutic modalities, and adaptive equipment
• Excellent communication, interpersonal, and problem-solving skills
• Ability to work both independently and collaboratively in a team environment
• Compassionate, patient-focused approach to care

Preferred Experience / Skills

• Experience working in a skilled nursing facility or similar healthcare setting preferred
