Key Responsibilities
- Provide compassionate nursing care to hospice patients in a home care setting
- Manage a caseload of approximately six patients daily, focusing on wounds and disease management
- Document patient progress and communicate with the clinical support team
- Adhere to department protocols and ensure compliance with licensing and certification requirements
- Complete required orientation including classroom and precepted training
Required Qualifications
- Valid IL LPN or LVN license; active or in-application status
- Current BLS certification
- Valid driver's license and insurance
- Must have wound vac experience
- Ability to produce a negative drug screen, as OSF operates a drug-free campus
- Last four digits of Social Security Number and middle initial for submission
Preferred Experience / Skills
- Experience with wound care and disease management in a home health environment
- Knowledge of Wound Vac systems
- Comfortable working independently with little supervision
