Key Responsibilities

• Provide physical therapy services to home health patients in accordance with physician orders and patient needs.
• Assess patient conditions and develop treatment plans to improve mobility and function.
• Document patient progress and treatment outcomes utilizing the designated documentation system.
• Collaborate with the healthcare team to coordinate patient care and ensure safety.
• Travel to patient locations, requiring reliable transportation.
• Work independently within assigned schedules to deliver quality patient care.

Required Qualifications

• Valid license to practice as a Physical Therapist in North Carolina.
• At least 2 years of experience in home health or hospice settings.
• Proven experience providing therapeutic services in a home health environment.
• Ability to travel within the Asheville/Franklin/Macon County area.
• Strong assessment and documentation skills.
• Reliable vehicle for patient visits.

Preferred Experience / Skills

• Experience working with adult and elderly populations.
• Familiarity with HCHB documentation system.
• Strong communication and organizational skills.
