Job Title
• Therapist - Occupational Therapist
Location
• Santa Rosa, CA
Schedule / Job Type
• Full-time, day shifts, 5 days per week
Key Responsibilities
• Conduct assessments and develop treatment plans for patients with acute and chronic disorders
• Implement therapeutic interventions to improve functional activities
• Participate in patient safety goals and core measures related to infection prevention and fall/risk assessments
• Document patient progress using electronic charting (Epic)
• Collaborate with interdisciplinary team members to ensure optimal patient care
• Facilitate patient education and discharge planning as needed
Required Qualifications
• Valid California Occupational Therapist license in hand
• Minimum of 1 year of relevant experience in an acute care setting
• Experience with Epic electronic medical record system and clinical charting
• Certification in Manual Therapy and Hand Therapy
• Ability to float between units and facilities as needed
• Willingness to work 2 out of 3 major holidays (Thanksgiving, Christmas, New Year)
Preferred Experience / Skills
• Experience working within a hospital environment with acute and chronic condition management
• Familiarity with infection prevention protocols and fall/risk assessments
• Previous experience working in a facility utilizing Epic charting system
