Schedule / Job Type
- Full-time, 8-hour day shifts, 5 days per week
Key Responsibilities
- Provide occupational therapy services to home health patients in West Portland and surrounding areas
- Assess patient needs and develop personalized treatment plans
- Document patient progress accurately using EPIC, Rover, and Oasis systems
- Collaborate with family members and caregivers to ensure optimal care and safety
- Travel to multiple patient homes within a 60-mile radius, minimizing drive time
Required Qualifications
- Valid Occupational Therapist license in Oregon
- BCLS certification
- Minimum of 1 year of skilled home health experience
- Own reliable transportation for traveling between patient locations
Preferred Experience / Skills
- Oasis documentation experience
- Previous home health or outpatient therapy setting experience
- Ability to coordinate and prioritize patient visits efficiently
