Key Responsibilities
- Provide skilled physical therapy services to home health patients in East Portland and surrounding areas.
- Assess patient needs, develop individualized treatment plans, and implement therapeutic interventions.
- Document patient progress and treatment activities using electronic documentation systems.
- Collaborate with caregivers and healthcare teams to ensure optimal patient outcomes.
- Travel within a designated radius to visit multiple patients daily, minimizing drive time.
Required Qualifications
- Active Physical Therapy license in Oregon or pending license accepted.
- BCLS certification.
- At least 1 year of experience in skilled home health therapy.
- Own transportation required; reliable vehicle needed for travel between patient homes.
Preferred Experience / Skills
- Experience with EPIC, Rover, or Oasis documentation systems.
- Ability to work independently in a home health setting.
- Strong organizational and time-management skills.
