Job Title
• Therapist - Physical Therapist

• Minot, ND

Schedule / Job Type
• Full-time, Days shifts, 40 hours/week (5x8 days)

Key Responsibilities
• Provide rehabilitative services to home health and hospice patients.
• Conduct assessments and develop treatment plans tailored to individual and family needs.
• Implement therapy treatments under medical direction, focusing on safety and rehabilitation.
• Educate patients and families on safety, care, and maintenance procedures.
• Coordinate therapy services and consult with other healthcare personnel as needed.
• Document patient progress and maintain compliance with healthcare regulations.

Required Qualifications
• Licensed Physical Therapist in North Dakota.
• Minimum of 2 years of previous experience as a Physical Therapist preferred.
• Current driver's license and available personal transportation to visit patients' homes.
• Ability to provide recent travel experience if applicable.
• Must submit a valid licensure report (Full NURSYS report if RN).

Preferred Experience / Skills
• Experience with Cerner electronic health records system strongly preferred.
• Prior experience working with geriatric populations in home health or hospice settings.
