Key Responsibilities
- Provide skilled physical therapy services to patients in a home health setting.
- Assess patient needs and develop personalized care plans.
- Document clinical notes using electronic health records systems such as EPIC and Rover.
- Collaborate with healthcare team members to ensure optimal patient outcomes.
- Travel between patient locations within a 60-mile radius, minimizing drive time.
Required Qualifications
- 1 year of skilled home health experience.
- Current and valid physical therapy license.
- BCLS certification.
- Experience with Oasis documentation system required.
Preferred Experience / Skills
- Prior experience in home health therapy.
- Familiarity with EPIC and Rover platforms.
- Reliable transportation and ability to travel within specified radius.
