Job Title

Schedule / Job Type

• Full-time, Night shift (12-hour shifts, 36 hours per week)

Key Responsibilities

• Provide skilled nursing care to patients in a Skilled Nursing Facility setting, collaborating with interdisciplinary teams.
• Administer prescribed medications and treatments following approved techniques, policies, and legal standards.
• Document patient care accurately and in compliance with standards of care.
• Develop and execute personalized care plans in coordination with physicians and patient families.
• Participate in department performance improvement initiatives and support shared governance activities.
• Utilize clinical expertise to support other units and departments in delivering comprehensive patient care.
• Apply critical thinking to ensure patient safety and quality of care, escalating concerns as appropriate.

Required Qualifications

• Current Wisconsin RN license.
• CPR certification.
• Ability to float between in-patient units based on census needs.

Preferred Experience / Skills

• Prior experience working in LTC or SNF environments.
• Strong knowledge of nursing practices and patient care procedures.
• Excellent communication and documentation skills.
