Key Responsibilities

• Provide skilled nursing care to home health patients in accordance with physician orders and care plans.
• Conduct patient assessments, monitor vital signs, and administer medications as prescribed.
• Educate patients and their families on health management and care instructions.
• Document care activities accurately using electronic charting systems, including Epic/Oasis.
• Coordinate with multidisciplinary teams to ensure comprehensive patient care.
• Manage a 1:1 patient ratio, visiting up to six patients per day, adhering to productivity standards.

Required Qualifications

• Valid state RN license, pending licenses accepted.
• Minimum of 1 year of skilled home health experience.
• BCLS certification.
• Own reliable transportation for travel to patient's homes.
• COVID-19 vaccination including primary series and one booster.

Preferred Experience / Skills

• Experience with Oasis documentation.
• Previous experience in a home health setting.
• Ability to work independently in a community-based environment.
